Эффективность и безопасность апиксабана: что изменилось за 12 лет его использования в реальной клинической практике?

Настоящий обзор рассматривает позиции апиксабана среди других прямых оральных антикоагулянтов у больных фибрилляцией предсердий (ФП) и венозными тромбозами в рандомизированных клинических исследованиях и реальной клинической практике. За время, прошедшее с момента появления апиксабана, появилось множество исследований реальной клинической практики (РКП) и метаанализов. Исследования РКП полностью подтвердили эффективность и безопасность апиксабана. Сейчас мы располагаем данными, что апиксабан одинаково эффективен и безопасен у лиц пожилого и старческого возраста и более молодых пациентов, также у больных со сниженным клиренсом креатинина. Эффективность и безопасность апиксабана подтвердились у пациентов с ФП с наличием заболеваний нескольких сосудистых бассейнов, у больных ФП и стенокардией, включая острый коронарный синдром, леченных путем стентирования коронарных артерий и консервативно. Важным свойством апиксабана является безопасность, прослеживаемая практически во всех исследованиях, позволившая с успехом применить его у лиц, имеющих высокий риск кровотечений, а именно у больных активным раком и венозными тромбозами. Таким образом, исследования реальной клинической практики у больных ФП, включая пожилых и больных старческого возраста, имеющих хроническую болезнь почек, массу сопутствующих заболеваний, а также пациентов с ВТЭО подтвердили результаты рандомизированных клинических исследований, касающихся апиксабана. Препарат выглядит как эффективный и в то же время наиболее безопасный антикоагулянт. Шлейф безопасности, наметившийся у апиксабана во всех РКИ (у больных ФП, ВТЭО, в т. ч. и у больных с активным раком), нашел свое подтверждение и в исследованиях реальной клинической практики, и метаанализах и позволил с успехом применять его у наиболее угрожаемой в отношении кровотечений категории больных.
